LICENSE/CERTIFICATION

  • Current RN license or temporary permit for practice in the state of North Dakota.

FUNCTIONS

  • Identify Medicare eligibility and qualifications of prospective skilled care residents, both prior to and during the residents stay in the facility.
  • Attend and conduct IDT meeting with appropriate department staff, residents, family members, and physicians.   Define the need for additional Care Conferences in collaboration with other health care team members.
  • Complete weekly chart audits of each Medicare resident; ensure completeness of documentation and the presence of supporting information for continued skilled care services.
  • Assure the facility is in Technical Compliance with requirements of a quality Medicare program. This includes timeliness, completion of and issuance of proper letters and notifications, certifications, re-certifications, documentation of Care Conferences held, and Care Plan updates.
  •  Assure timely completion and submission of the MDS according to OBRA and PPS Schedules.
  • Provide quality assessment of each resident. This Includes review of preadmission assessments and all subsequent required assessments to ensure continued payor qualifications are met, and services being provided are adequate to resident’s needs.
  • Responsible for developing a plan of care for each skilled resident, based on the MDS, with the collaboration of the appropriate departments to achieve this plan.
  •  Collaborate with Social Services and the Business Office as a contact between the facility and payor, providing pertinent information in regard to the resident’s status and needs, to ensure reimbursement.
  • Assure that each neighborhood is in compliance with specific MDS related documentation, submissions, and transmissions.
  • Identify specific needs, items, or equipment pertinent to the care of skilled residents to maintain or improve the highest practical level of wellbeing.
  • Ensure that information transmitted to payor is accurate and complete as based on the proper and supporting documentation provided by the disciplines involved with a resident’s plan of care.
  • Assure continuity between services being provided, nursing assessment, notes, and charting of various departments and physician orders.
  • Responsible for addressing unsatisfactory immediate care related issues or staff performances, as reported and observed.
