LICENSE/CERTIFICATION
- Current RN license or temporary permit for practice in the state of North Dakota.
FUNCTIONS
- Establish regular and ongoing communication with the acute care and home care staff in the community to promote referrals to Valley Senior Living facilities.
- Conduct pre-admission screening of potential residents to assess resident needs.
- Assess whether the Skilled Nursing Facility has the skills and resources to provide the care required.
- Determine whether the Skilled Nursing Facility Admission would be Medicare A covered.
- Serve as liaison/communicator with the nursing admitting staff in the Skilled Nursing Facility or Home Care/Assisted Living facility.
- Work with the Admissions Coordinator regarding each admission to the Skilled Nursing Facility to confirm room placement, and/or roommate compatibility.
- Review the referral form from the referring agency/facility regarding primary diagnosis, medications, treatments and functional capacity of the patient/resident.
- Request and attend discharge conferences as necessary to ensure continuity of care for residents.
- Communicate with the Inter-Disciplinary Team (IDT) concerning special needs of residents i.e. equipment, dietary concerns, behavioral, emotional concerns.
- Assist with/conduct DDM/Ascend Level I//Level II/Level of care screening required for ND nursing home placement.
- Effectively make arrangements for new admits in the absence of the Admissions Coordinator; communicate with referring facilities and Valley Senior Living staff.
