Revised Mission, Vision, Values, and Code of Conduct!
As we enter the next century of providing care and service to the community, we are excited to share some important updates that reflect our evolving focus on the needs of our residents and employees. After thoughtful deliberation by the Board of Directors and leadership teams through strategic planning for our future, we have revised our Mission, Vision, and Values to ensure they align with our desire to provide compassionate Christian care for those we serve.
Why Are These Updates Important?
These updates are not just about refining our guiding principles—they are a reflection of the evolving needs of both our residents and employees. As we continue to grow, we must ensure that our organization’s culture and actions align with our core values. This includes revisiting our Code of Conduct to ensure all team members are clear on their responsibilities and understand the expectations.
Introducing the Updated Code of Conduct
The Code of Conduct serves as a foundational guide for how we interact with one another, with residents, and with the broader community. It clarifies expectations and ensures that we uphold the values of relationships, empowerment, and excellence.
Next Steps
We ask each care partner to carefully review the revised Mission, Vision, Values, and Code of Conduct. To help ensure everyone is aligned, we will continue to share the actions associated with our values through our corporate culture education and throughout the first quarter of 2025 in Quality of Life and Quality of Care meetings.
All employees will sign the updated Code of Conduct in the near future.

Update on UKG Timeclock Rounding
Last week we communicated that with the new timeclock rounding, employees would be able to punch in up to 3 minutes before their scheduled shift. This information was incorrect – the punch must be within 2 minutes of the scheduled start time. Sorry for any confusion this has caused. Nominations for Care Partner of the Quarter
There’s still time to nominate somebody for Care Partner of the Quarter! Some considerations for who to nominate include someone who:
- Exceeds expectations of other care partners including residents, families, and staff
- Builds relationships through caring and accountability
- Is a team player
Nomination boxes are on each neighborhood and nominations will be accepted through November 27. Take the time to recognize the care partners on your neighborhood!
QAPI Update: F609 Reporting of Alleged Violations
F609 states: Facility leaders need to ensure timely reporting of all alleged violations involving abuse, neglect, exploitation, or mistreatment, including injuries of unknown source and misappropriation of resident property.
What you need to know: A facility must develop and implement written policies and procedures that ensure timely reporting of all alleged violations. Reporting of any suspected abuse or incident involving serious bodily injury must be done no later than 2 hours from the incident. Any other alleged violations must be reported within 24 hours of the incident. An investigation of the allegation must be concluded within 5 business days of the initial report. The facility must also provide annual training on abuse, neglect, mistreatment, and exploitation. If you witness or suspect abuse, neglect, mistreatment or exploitation, you must report this to your supervisor immediately.
Culture Connection of the Week
The language we use to describe people and places has a huge influence on the environment around us. This month we are focusing on the words we hear and use, and how we can improve our language to create an environment that is less like an institution and more like a home.
What you need to know: Dr. Bill Thomas, founder of The Eden Alternative, says that institutional language can be the food that feeds the ever-present dragon that is waiting to re-establish the institutional model of care and destroy all efforts to shift the culture of care to improving well-being of all. The Eden Approach calls on us to adopt language that can change attitudes, beliefs, and behaviors to support person-directed care.

Access UKG Feed for Additional Information
We have started to utilize the UKG employee communication tools within the UKG app and on the UKG desktop login. When you log in to the UKG app, navigate to the “FEED” option at the bottom of your screen. Using the desktop version, log in to your UKG Pro account, click “MY COMPANY” then “COMMUNICATIONS.” You will find company news and information, upcoming events, and a space to communicate with supervisors and co-workers.

Open Enrollment for Benefits Begins on November 17!
The Open Enrollment period will run from November 17 to November 30, 2024. During this time, full time and part time benefited employees will have the opportunity to review and adjust their benefits for the coming year through the UKG system. For those of you who are eligible for benefits, click HERE to view instructions on how to access the module through UKG. You will need to utilize a web browser to enroll – you cannot enroll through the app. It would be beneficial for you to bookmark the UKG website (https://valleyonukg.ukg.net/) on your phone so you can still use your phone to enroll.

Remember, Open Enrollment is the only time each year to make changes to your benefits outside of a qualifying life event. UKG Updates – Timeclock Rounding and Submitting UKG Issues
Timeclock Rounding – We previously announced the new timeclock rounding process from 15-minute rounding to 6-minute rounding. You will notice that your timecard now shows tenths of an hour (0.1, 0.2, etc) rather than quarters of an hour (0.25, 0.5, etc). With this process change, you now must punch in or out 3 minutes before or after your scheduled shift to be within your scheduled start and end times. For example, if you start your shift at 5:45 am please do not punch in before 5:42 am. If your shift ends at 5:00 pm, please do your best to punch out before 5:03 pm.
Submitting UKG Related Issues – each desktop and kiosk across the company now has a shortcut where you can submit any UKG related issues, such as password resets, issues finding an employee you supervise, etc. To submit an issue, click on the icon labeled “UKG Issues” and an email will pop up for you to fill in which will then be sent to HR to assist in resolving the issue.

The Valley Senior Living Foundation, in cooperation with Human Resources, is now accepting applications for the Greg V. Hanson Scholarship (up to $15,000) through December 31, 2024. Eligibility criteria for this scholarship include:
- Must be a regular status employee to apply (flex employees are not eligible)
- Must have been employed with Valley Senior Living for an accumulated total of at least three years
- Not currently enrolled in post-secondary education
- Satisfactory performance evaluations and the ability to carry out the mission of Valley Senior Living

Woodside Village Featured for North Dakota’s Quality Incentive Program
Woodside Village was recently featured by the North Dakota Department of Health and Human Services for our efforts in reducing antipsychotic medications using a collaborative approach with the interdisciplinary team. Click HERE to read the article.
QAPI Update: F582 Medicaid/Medicare Coverage/Liability Notice
F582 states: The facility must inform each resident before, or at the time of admission, and periodically during the resident’s stay, of services available in the facility and of charges for those services, including any charges for services not covered under Medicare/ Medicaid or by the facility’s per diem rate.
What you need to know: There is a rate setting guide that spells out services that the facility can or cannot charge for. The facility cannot charge a resident for certain over-the-counter medications, meals, or laundry services as those are included in the resident’s daily rate. Any time the facility makes changes to the services they provide, a 60-day notice must be issued in writing prior to the change. Examples would be an increase in beauty shop charges or room rates.

Way Back Wednesday – 100 Years of Caring
On November 7, 1992, the Bethesda Society held their 27th Annual Bazaar and Luncheon at the Valley Memorial Home on Almonte Avenue. The Bazaar consisted of events from 9:30 am to 4:30 pm including lunch, a bake sale, quilts, afternoon pie and coffee, white elephant gifts, and more. Funds raised by the Bethesda Society were given to Valley. In a letter from Mary Ellen Preston, Facility Coordinator, in December 1992, she states that “The Bethesda gift has made the first impression of Almonte a bright and welcoming one…This is indeed a blessing to residents, their families, staff and visitors.” We continue to be grateful for all that the Bethesda Society contributed in their 100 years of service to make Valley a comfortable, warm, and cheerful home to all who live here.
UKG Updates
Punching In – On November 1, we started utilizing UKG to punch in and out. Staff now have the ability to punch in via several options:
The UKG Pro App – this is the easiest option as you can punch in right from your phone! Please do not punch in on your phone until you are in your work area.
- The timeclock kiosks at each community
- A neighborhood kiosk – 1 kiosk has been identified on each neighborhood to house the UKG WFM shortcut to punch in and out.
- Additionally, we are working on making it so that those who work directly from a computer will be able to clock in and out via a desktop format.
Pay Period Review – You can now review your hours worked for this current pay period within the UKG App – be sure to look for the “My Timecard” tile and move to the correct pay period.
PTO/ESL – We anticipate that you should be able to view your PTO/ESL balances within the UKG system on Thursday. Once populated, you can then begin submitting your PTO requests in the UKG app.
Attendance – Later this week, we will begin the export/import process for attendance points from AOD to UKG.
Process for submitting issues regarding UKG – HR is currently in the process of establishing a process that would allow staff to submit their issue with UKG to an email address such as password issues, not seeing the appropriate department to transfer to, etc. A member of the HR department would then receive this and begin working on resolving the issue for the staff member.

We look forward to providing you with a connected system where you will be able to punch in, view schedules, submit and view time-off requests, adjust schedules when needed and swap shifts, track performance, check paycheck accuracy, sign up for benefits, and more all in one location.

QAPI Update: F623 Notice Requirements Before Transfer/Discharge (Part 2)
F623 states that: For transfers or discharges of a resident, prior to the transfer or discharge, the facility must notify the resident and the resident’s representative(s) of the transfer or discharge and the reasons for the move in writing and in a language and manner they understand. Additionally, the facility must send a copy of the notice of transfer or discharge to the representative of the Office of the State Long-Term Care (LTC) Ombudsman.
“Facility-initiated transfer or discharge” is not initiated by the resident and may not be the resident’s goal and preference.
What you need to know: The most common type of transfer takes place when a resident has a need that cannot be met by the nursing home, such as in the case of a transfer to the ER/hospital for an acute infection or condition. In rare cases, a 30-day discharge notice is issued to the resident due to clinical or behavioral issues that endanger the health and/or safety of others. In either case, accurate CNA and nursing documentation is important. The Notice of Resident Transfer, Discharge or Hospitalization must be completed at the time of discharge and provided to the resident and/or PDM.

UKG – Friday start date for timeclock punches!
Beginning Friday, November 1 at 12:00 AM, we will say HELLO to UKG and employees will begin to punch in and out through UKG Workforce Management. Employees are encouraged to use the UKG mobile app on your phones, with additional options including the UKG desktop version, or specific kiosks in each neighborhood and community.
Things to remember before your first punch and how to prepare for Friday:
- Log in to your UKG Pro App to ensure you can view your schedule and see the punch in/out option.

- Turn ON the LOCATIONS within the app so you can punch in once you arrive at work.

UKG – New Rounding Practice for Timeclock Punches
On November 1, when Valley starts punching in through UKG and the new timeclock management system UKG Workforce Management you may notice some changes in how your punch time is rounded for pay. As part of the transition, Valley will be moving from a 15 minute rounding practice to a 6 minute rounding practice which is essential to meet CMS PBJ reporting requirements. What does this mean? Your timecard may look different – you will see tenths of hours (ex. 0.1, 0.2, 0.3, etc.) and you will no longer see quarters of hours of 0.25 or 0.75 on your timecards.
UKG TALK – A feature of UKG PRO that Expands Company Communication
We have started to utilize the UKG employee communication tools within the UKG app and on the UKG desktop login. When you log in to the UKG app, navigate to the “FEED” option at the bottom of your screen. ND Long Term Care Association Opposes Measure 4
The ND Long Term Care Association, ND Medical Association, ND Hospital Association, and others within healthcare oppose Measure 4. The projected effect of no property tax would be over a $3 billion shortfall per biennium which would increase other taxes and fees or decrease program support. The Department of Health and Human Services budget is the largest budget in the state. Measure 4 could result in decreased funding for long term care services and impact our ability to care for residents. More information can be found on at https://www.keepitlocalnd.org/. The Grand Forks Chamber of Commerce recently shared this LETTER to the Grand Forks Herald from the Grand Forks Mayor, Brandon Bochenski.

“Resident-initiated transfer or discharge” is requested by the resident or primary decision maker and the intent is that the resident will be leaving the facility without planning to return.
What you need to know: This is the most common type of discharge and is seen most frequently on Valley Transitions. Upon move-in, plans are already in motion for the resident’s discharge to a lesser level of care, whether it be to assisted living, basic care or independent/home. Accurate CNA and nursing documentation is important to make sure discharge goals are realistic and discharge planning can proceed. Weekly Reflection: During your ACE session in August, you were asked to identify your wobble and make a commitment to improve one behavior associated with your wobble. Reflect on the behavior you committed to improve. Are you making progress? Do you need to make changes to meet your goal? Is there a new behavior you can commit to improving? Are you relying on your trusted colleague to help hold you accountable?

ND Long Term Care Association Opposes Measure 4
The projected effect of no property tax would be over a $3 billion shortfall per biennium which would increase other taxes and fees or decrease program support. The Department of Health and Human Services budget is the largest budget in the state. Measure 4 could result in decreased funding for long term care services and impact our ability to care for residents. The ND Medical Association, ND Hospital Association, and others within healthcare oppose this measure. More information can be found on at https://www.keepitlocalnd.org/.
QAPI Update: F603 Free from Involuntary Seclusion
F603 states residents have the right to be free from involuntary seclusion. Involuntary seclusion is the separation of a resident from other residents or confined to their room against their will.
What you need to know: Staff are not permitted to confine residents to an area without access to a call light or other methods of communication, including placing the call light out of reach of the resident. Staff are not permitted to block doorways to prevent residents from leaving an area, including shutting neighborhood doors to deter a resident from leaving the neighborhood. Isolation due to Transmission-based Precautions or placement on a secured neighborhood is not considered involuntary seclusion as the medical record and facility policies support these instances.

What you need to know: There are 3 fundamentals of trust – Empathy, Logic, and Authenticity. This week we will look at the authenticity wobble. Being authentic is to be yourself. Resist the urge to hold back who you are. When you hold back, you are less likely to be consistent, therefore less likely to be trusted.
Weekly Reflection: Think of ways you can improve your authenticity wobble. Be yourself and walk the talk. Pay less attention to what you think others want to hear from you and say what your true self needs to say.

Happy 100th Valleyversary to us!
On October 15, 1924, Mrs. Swenson, Mrs. Nelson, Mr. and Mrs. Myrah, and Mrs. Jaran Leen were the first residents, then known as the Grand Forks Home for Aged, or sometimes referred to as the O.P.H. -the “Old People’s Home” on Almonte Avenue.
The original admission application excerpt below is from Mrs. Carrie Swenson, our first resident. Carrie was born in Norway in 1852, was married, and had been widowed five years prior to moving in. She longed for “a peaceful place to stay.”
It is humbling to think about our beginnings and what we have done for the people in our community over the last 100 years! Thank you all for your part in making Valley a home, “a peaceful place to stay,” for the residents!

QAPI Update – F Tag of the Week: F600 Free from Abuse and Neglect (Part 6)
F600 states that residents have the right to be free from abuse and neglect. Neglect is defined as failure by the facility or employees to provide goods and services necessary to avoid physical harm or pain, mental anguish, or emotional distress. Failure to provide the necessary care and services to a resident may not only have a negative physical outcome but could result in a negative psychosocial outcome for the resident.
What you need to know: The facility has a responsibility to provide sufficient training, supplies, and knowledge on how to care for each resident. Staff failure to utilize these resources may result in a neglect allegation. For alleged violations of neglect that result in serious bodily injury, the facility must report the allegation immediately and no later than 2 hours after the allegation is made (example: Care Plan not followed results in a fall with a fracture, head injury, etc). For alleged violations of neglect WITHOUT serious bodily injury, the facility must report the allegation within 24 hours (example: resident not assisted to the bathroom according to their Care Plan, resulting in incontinence, reddened skin and/or feelings of humiliation). Any allegations of abuse (physical, mental/verbal, or sexual) must be reported to your supervisor immediately. This includes allegations made against staff, family members, other visitors, volunteers, contracted staff, or residents.

100 Years of Caring on Tuesday, October 15! – Way Back Wednesday
For the past 100 years Valley Senior Living has provided care and services in Grand Forks. The original ‘Home for the Aged’ pictured below was founded by local Lutheran churches and women’s church groups and opened on October 15, 1924. From our humble beginnings on Almonte Avenue with room for 40 residents to the three care communities and the capacity to care for nearly 550 residents and employ 850 staff today, Valley Senior Living is woven into the fabric of the Grand Forks community.
This milestone of 100 years of caring for residents would not have been possible without the continued commitment of the local Lutheran churches that govern the organization, church groups including the Bethesda Society that worked tirelessly to raise funds, employees, volunteers, and those that have entrusted us to provide compassionate care to thousands of individuals.
Thank you for being a part of Valley Senior Living’s 100 years of caring for loved ones in our community! We are looking forward to the next century and beyond. Please join us next week as we celebrate. Each celebration will include a short program followed by fellowship with cake and ice cream. Join us at any or all of the celebrations:

Nursing Facility Incentive Program
North Dakota Health and Human Services has a new quality care initiative aimed at improving overall experiences and outcomes for residents of nursing facilities. The Nursing Facility Incentive program provides an annual payment to facilities based on their performance in four CMS resident care and facility process measures: urinary tract infection, antipsychotic drugs, pressure ulcers, and hospitalizations. This year, $4 million in incentive payments were distributed to across four tiers to 58 nursing facilities across the state.
Congratulations to Woodside Village as one of four nursing homes in the state to earn top tier status and congratulations to Valley on Columbia was one of 28 facilities to earn tier two status in the first year of the program! To learn more about the Nursing Facility Incentive program, click here.

F600 states that residents have the right to be free from abuse and neglect. Mental abuse includes both verbal and nonverbal actions which cause – or have the potential to cause – a resident to experience intimidation or fear, shame, humiliation, degradation or agitation.
What you need to know: Verbal abuse, which includes oral, written, or gestured communication or sounds, can also be considered mental abuse when used within hearing distance of residents. Keeping or distributing photographs or recordings of a resident that demeans or humiliates the resident can be considered abuse even if the resident has provided consent and regardless of whether they are cognitively intact. Any allegations of abuse (physical, mental/verbal, or sexual) must be reported to your supervisor immediately. This includes allegations made against staff, family members, other visitors, volunteers, contracted staff, or residents.

Many statewide and local organizations are deeply concerned about the potential impact on the state’s services if Measure 4 passes in North Dakota at the November election. The projected effect of no property tax would be over a $3 billion shortfall per biennium. Should Measure 4 be approved, Medicaid is likely to face significant reductions. Ultimately, that would result in decreased funding for long term care services and impact our ability to care for residents. Please consider voting NO on Measure 4 in November’s election. More information can be found on the Keep it Local website.
Do you rent? If you don’t own property, a YES vote will increase your payments as new taxes need to be added. If property tax goes away, how likely is it that your landlord will decrease your rent? Pretty unlikely. Meanwhile, the state needs to make up for the shortfall somewhere – sales tax, income tax, “fees” that are a tax, and those taxes will certainly find you, making Measure 4 even more problematic.

What you need to know: Sexual contact is considered to be non-consensual under several circumstances, including if a resident appears to want the contact but is unable to consent, if the resident does not want the contact to occur, and if the resident is sedated/unconscious/in a coma. If you witness/suspect sexual abuse or a resident alleges sexual abuse you must report to your supervisor immediately. REMINDER: Any allegations of abuse (physical, mental/verbal, or sexual) must be reported to your supervisor immediately. This includes resident allegations made against staff, family members, other visitors, volunteers, contracted staff, or residents.

100 Years of Caring – Way Back Wednesday
The flood of 1997 was a was a devastating time for our organization. The dike holding the flood waters back broke on April 18, residents were evacuated, and the original building at 1023 Almonte Ave was flooded. With the support of leadership and the community, quick action was taken to be able to break ground on September 22, 1997 at 4000 24th Ave S, now known as Valley Senior Living on 42nd, just five months after the flood. Valley Senior Living on 42nd provided a new home to residents who were evacuated from 1023 Almonte Ave and remains the only retirement living community with a full continuum of care in the Grand Forks area.

ND Long Term Care Association Encourages Vote NO on Measure 4
Many people are deeply concerned about the potential impact on the state’s services if Measure 4 passes in North Dakota at the November election. The projected effect of no property tax would be at least a $2.6 billion budget shortfall per biennium. Should Measure 4 be approved, both traditional Medicaid and Medicaid Expansion reimbursements are likely to face significant reductions. Ultimately, that would result in decreased funding for long term care services and impact our ability to care for residents. Please consider voting NO on Measure 4 in November’s election. QAPI Update – F Tag of the Week: F600 Free from Abuse and Neglect (Part 3)
F600 states that residents have the right to be free from abuse and neglect including physical abuse. Physical abuse includes actions such as hitting, biting and slapping, as well as corporal punishment. Corporal punishment is meant to correct or control the behavior of a resident and involves someone taking an action such as pinching, flicking, slapping hands or hitting with an object.
What you need to know: Physical abuse towards a combative resident IS NOT an appropriate response in any situation. It is also not acceptable for an employee to claim their action was “reflexive” or a “knee-jerk reaction” and was not intended to cause harm. Retaliation by staff is abuse, regardless of whether harm was intended. If you witness or suspect physical abuse you must report to your supervisor immediately. This includes resident to resident interactions.

It is through the relationships we have with one another that we can discover ways to recognize and celebrate moments of joy when they occur. Joy flourishes because care partners are relaxed with one another.

During ACE sessions in August, everyone had the opportunity to review the Code of Conduct and provide feedback on statements that were the most important as well as suggestions for improvement. The feedback from the 580 surveys was used to update the Code of Conduct for all Valley employees. The Code of Conduct provides direction for our organization and outlines expectations for employees. The Wellness Committee invites you to attend the TEARS Talk & Walk on September 22 at the Ralph Engelstad Arena. Registration begins at 12:30 PM, the welcome speaker will begin at 1:30 PM, and the 1-mile walk begins at 2:00 PM. The TEARS organization has a mission to create hope, build strength, and end suicide. You are welcome to wear a Valley Senior Living shirt and support this cause.

If staff have the knowledge and ability to provide the necessary care and services to ensure a resident attains or maintains the highest well-being, but chooses not to, this is considered abuse. It is also considered abuse if a staff member does not acknowledge a request for assistance that results in a negative outcome.
What you need to know: examples of deprivation of services may include not taking someone to the bathroom when requested so they become incontinent, or withholding care, treatments, or medications thereby exposing that person to the risk of physical, mental, or emotional harm. If you witness or suspect deprivation or services, you must report to your supervisor immediately.

100 Years of Caring – Happy Assisted Living Week!
In August 1988, Tufte Manor held a mortgage burning ceremony and celebrated the achievement of fulfilling all financial commitments attached to the mortgage.
This week is recognized as National Assisted Living Week. Valley Senior Living has provided assisted living options in Grand Forks for many years. Tufte Manor originally opened as an independent living community in 1970 and later changed to assisted living followed by the current basic care offering. When 4000 Valley Square was built in 1998 to replace the original Almonte Avenue location after the flood of 1997, assisted living was included as part of the retirement community that was developed. Valley is proud of the assisted living care options we have had throughout the years and the options we continue to offer the community.

100 Years of Caring
Harvest Festivals were a tradition at Valley Memorial Homes. The 10th Annual Harvest Festival was held on October 17, 1965, with lunch served by the Bethesda Society. During the Harvest Festivals, funds were raised to support the home and appeals were made to supporting congregations to provide a Harvest Festival offering to allow the home to provide a Chaplain for residents.

Valley Senior Living Medical Provider Changes
There are big changes happening in September with our group of on-site Altru providers! Erica Hanson, Nurse Practitioner, will soon be moving to Georgia to be closer to her family. Her last day with us will be on Thursday, September 5. Erica has dedicated her career caring for residents at Valley for over 25 years and we wish her the best in the future!
In September we are welcoming two new Altru providers who will be making weekly visits onsite at Valley Senior Living: Dr. Sarah Weisenberger and Dr. Taylor Volberding.
Dr. Sarah Weisenberger will see residents on Thursday afternoons at Valley on Columbia starting on September 5. Dr. Weisenberger is a native of West Fargo and a new family medicine physician at Altru. She provides primary care services for all ages. In her free time, she enjoys spending time with her family and friends. She also enjoys going for walks with her dog and trying new recipes. What she loves most about medicine is how it is centered around individual patients. Her philosophy of medicine is to meet people where they are, show compassion, and assist in any way she can. She feels rewarded by seeing patients have positive outcomes in response to the care she has given.
Dr. Taylor Volberding will see residents on Monday afternoons at Woodside Village starting on September 22. Dr. Volberding is a native of Fargo and a new family medicine physician at Altru. She first became interested in medicine when she was a nurse assistant at a long-term care facility in Fargo. She is ecstatic to continue with geriatric care in her family medicine practice. Dr. Volberding and her husband have a busy household with a daughter and a dog. They love to explore the National Parks and find every little coffee shop along the way.
While September will bring much change in our group of providers, we wish a fond farewell to Erica Hanson and welcome Dr. Weisenberger and Dr. Volberding

Healthcare Academy Modules Open on September 1
On September 1, all staff will be assigned Annual Healthcare Academy modules. All assigned Healthcare Academy modules must be completed by October 31. If you have not yet established your UKG account, please visit with a member of the HR team to assist in activating your UKG account.
Code of Conduct Survey Results
Thank you for your help with revising our Code of Conduct! During ACE sessions, employees were asked to complete a survey to learn more about what statements in the current Code of Conduct have importance to employees and what statements do not make as big of an impact. 580 surveys were completed and valuable feedback was provided.
When asked about each question individually, the following statements were the most important:
- I will recognize the worth in each person and treat each person as an individual.
- I will put the needs of the resident first.
- I will make everyone feel welcome.
- I will be an advocate for our residents.
- I will admit mistakes and take corrective action.
When staff were asked to vote on their top 3 statements, the following statements were the most important:
- I will recognize the worth in each person and treat each person as an individual.
- I will put the needs of the resident first.
- I will provide compassionate Christian care through the virtue of grace which is care that is given without expecting anything in return.
- I will be an advocate for our residents.
- I will recognize that our residents do not live where we work, we work in their home.
The most important statements will remain in the updated Code of Conduct. Several statements will be eliminated due to not being important when considered among other statements. Some statements will be removed or reworked due to redundancy and our desire to create a clear and concise Code of Conduct.
There were a number of comments that the statement about compassionate Christian care through grace was requiring staff to be Christian to work at Valley. That is not the case. We will clarify that in the revised code of conduct. We are just asking you to believe in the concept of grace, which is care that is given without expecting anything in return.
Again, thanks for your help with creating the code of conduct for our organization! Revisions are underway to a revised Code of Conduct with your input.
Join Valley in the 54th Potato Bowl Parade
Valley Senior Living will be celebrating 100 years of caring for the community with an entry in the 54th Annual Potato Bowl Parade on September 14 in downtown Grand Forks. Join the fun and help us celebrate a century of caring by walking in the parade. Participants will join our entry in line prior to 10 AM and walk down DeMers Avenue alongside the Valley van while passing out candy. 100 Years of Caring
The Valley Memorial Home at 1023 Almonte Avenue expanded in 1958 to add a nursing wing to care for 56 additional residents. This project and new service was made possible through contributions from community members and supplemented by Hill-Burton funds. QAPI Update: Reduced Coffee/Hot Water/Cocoa Temperatures
Today throughout the long-term care communities we lowered the dispensing temperatures for hot liquids to a serving temperature of around 140 degrees. Temperature lowering kits were installed on all coffee machines and the hot water function on water machines has been disabled. Reducing the possibility for serious burns has been a focus from the North Dakota Department of Health and Human Services as indicated by several high-level deficiencies cited due to burns to residents caused by coffee serving temperatures. The Health Department has shared that serious burns can occur in one second at 155 degrees and in five seconds at 140 degrees. Coffee and other hot liquids should be served at the temperature they are dispensed from the machine – do not microwave or otherwise attempt to raise the temperature of the dispensed liquids. If residents are concerned about the temperature being too cold upon serving, pour a new cup from the machine. Thanks for your understanding as we try to prevent these injuries from occurring.

Happenings and Upcoming Events
- Valley Transitions at Valley on Columbia is hosting a school supply drive for the Grand Forks Head Start classroom at Valley on 42nd. Donations will be accepted through August 21. Crayons, markers, assorted ziploc bags, glue, scissors, tissues, and backpacks are needed. Monetary donations will be accepted to purchase some larger items needed in the classroom.
- The Oakcrest neighborhood at Valley on 42nd is hosting a community wide Quarter War to raise funds that will be donated to the Grand Forks Public School to be used for at risk homeless youth to enable families to do laundry at local laundromats. In addition to quarters for laundry, laundry soap and supplies will be accepted. The quarter war will end on August 23 and the neighborhood that raises the most money will win a pizza party compliments of the Oakcrest neighborhood.
- Valley on Columbia will host the Dog Days of Summer Carnival in the front parking lot on Thursday, August 22 from 6:30 – 8 PM. Family, friends, and employees are invited to enjoy snacks and games together with residents.

QAPI Update: F602 Free from Misappropriation/Exploitation
F602 states that residents have the right to be free from misappropriation of their property and free from exploitation. Misappropriation is the deliberate misplacement or wrongful use of a resident’s belongings without the resident’s consent including money, clothing, jewelry, electronics, or personal information of residents including credit card information, bank account information, and Social Security cards. Exploitation is taking advantage of a resident for personal gain through manipulation, intimidation, threatening the resident or through coercion including asking residents to borrow money or personal items, or gaining access to resident’s personal holdings through persuasion, coercion, or solicitation.
What you need to know: our gift policy was updated to address borrowing from residents. Borrowing any items from residents is not permitted. Exceptions can be made with supervisor approval. Over the counter medications from the med cart are considered resident property and are not to be used by staff. We now have a staff supply of frequently requested over the counter meds available on each neighborhood. Failure to follow this can result in an investigation.

Planning for the Future
Throughout the past several months, Valley has been meeting and discussing a strategic plan which includes meeting with focus groups from each of the communities. It became clear during these discussions that we need to start planning for a new building in the future. While in conversation about future developments, a piece of land became available to purchase, and Valley Senior Living made its first land purchase since 1997! Discussion is ongoing with Altru to acquire more land to make building on this land possible. We do not have plans of building on the property anytime soon, but we are excited to share with you this big piece of Valley news!
100 Years of Caring – Way Back Wednesday
With the recent passing of former Grand Forks Mayor Pat Owens, we fondly remember her strong leadership and steadfast support of Valley Senior Living during the historic flood of 1997. The original Valley Memorial Home at 1023 Almonte Avenue was overtaken by flood waters while residents were evacuated and relocated to care communities across the state. Under the leadership of President/CEO Wayne Stark, and with the support of our corporate congregations and the Grand Forks community leadership which included Mayor Pat Owens, Valley Memorial Homes quickly determined a new care community would be built along 42nd Street South. The groundbreaking ceremony was held on September 22, 1997, and the current Valley Senior Living on 42nd opened to residents in the fall of 1998. The flood was devastating, but it strengthened our organization and helped to create what Valley Senior Living is today.

ACE Training Begins This Week
July 31 is the first day of Annual Corporate Education (ACE) sessions at the Alerus Center, 1200 S. 42nd St. Sessions will be held on July 31, August 1, 6, and 7. It is a yearly requirement for all employees to attend one of the sessions. Please contact HR, your supervisor, or scheduling if you are unsure which session you are scheduled to attend.

QAPI Update – F583 Personal Privacy/Confidentiality of Records
This F-Tag covers a wide range of residents’ rights related to privacy and confidentiality. The portion we are focusing on today relates to the right to secure and confidential personal/medical records and the safeguarding of that information. Though Valley on Columbia didn’t receive this deficiency, there was an instance during the survey of a med cart being left unattended with Point Click Care open and resident information visible.
What you need to know: Any resident information, either on a computer or on paper, should not be left unattended. Anytime you step away from a computer, you must either lock the computer or ensure you are logged out of Point Click Care. This ensures the privacy and confidentiality of resident information. If you have questions or concerns, contact the HIPAA Privacy Officer – Shannon Holte at Woodside Village, Susie Blue at Valley on Columbia, and Chery Ekren at Wheatland/Tufte.
Wellness Challenge Updates
The July Hydration Wellness Challenge ends today! Submit your hydration tracker to HR by August 9 for a chance to win the Visa gift cards.
The August Wellness Challenge is a sleep challenge to help transform your sleeping habits and boost your mental health. The challenge is to get a cumulative 7 to 9 hours of sleep per day for as many days as you can. Submit the SLEEP TRACKER with 18 or more nights of 7 or more hours of sleep per day and you will be entered to win a $150 VISA gift card or one of four $50 VISA gift cards. Submissions for the sleep challenge must be received by September 6.

What you need to know: The medical model of care strips away meaning in many different ways. The rhythm of daily life for residents can become repetitive and numbing if we don’t work to weave meaning into each resident’s day. Creating meaning takes effort and each of us can play a role.

QAPI Update – F880 Infection Prevention and Control – Part 2
As a reminder, F880 states the facility must establish and maintain an infection prevention and control program designed to provide a safe, sanitary, and comfortable environment, and to help prevent the development and transmission of communicable diseases and infections. The second part of the deficiency cited at Valley on Columbia was related to improper emptying of a urine drainage bag. In this specific observation, a barrier was not used between the container and resting surfaces, and the spout was not disinfected following drainage.
What you need to know: when emptying a urine collection bag, all steps must be followed in order to prevent possible transmission of diseases/infections. If you have any questions regarding this skill, please talk to the Charge Nurse, CNA Mentor, or Staff Development Nurse.

100 Years of Caring – Way Back Wednesday
The Bethesda Society was instrumental in the formation of our original care home at 1023 Almonte Avenue and provided constant support through our organization’s growth over many decades. The first ice cream social on the home lawn was held July 9, 1967, with Mrs. Carol Neilson, President, in charge and overseeing the event. Seventeen cakes and 3.5 gallons of ice cream were used netting a profit of $61.00 for the home.

Hi to everyone on a beautiful summer day! We need all staff to do two things if you have not done so already:
Claim your UKG account with the linked instructions: Click HERE to view the instructions if you have not already completed your account set up for UKG. Setting up your UKG account as soon as possible will ensure that you have access to your payroll information when it becomes available later this week
There is limited functionality to start with – only “pay” will be active. We have worked with UKG to create an email to all employees when your paycheck becomes available. Prior paychecks will not be visible within UKG, but they will continue be available on the Payroll Professionals website.
We will move everything over to UKG eventually, this is just the first step. You will continue to use AOD for time and attendance and vacation requests for the next few weeks. If you have not done so, please view the Healthcare Academy module assigned to you to learn more about UKG.

Welcome to Our New CNA Class!
This Monday a new CNA class with 20 students began their training through Valley to become Certified Nursing Assistants. We are excited to have this group of care partners join the team! QAPI Update – F690 Bowel/Bladder Incontinence, Catheter, UTI
F690 states the facility must ensure that residents who are continent of bladder and/or bowel receive services and assistance to maintain continence. A resident who is incontinent of bladder and/or bowel receives appropriate treatment and services to maintain dignity and hygiene and prevent urinary tract infections and skin breakdown.
What you need to know: Residents should be assisted to the bathroom every 2-3 hours, unless care planned otherwise. Some residents have a toileting schedule and should be assisted to the bathroom at their designated time frames as care planned. If a resident declines assistance to the bathroom when offered, this should be documented. If a resident continues to decline assistance to the bathroom, the charge nurse should be notified. Any changes in continence should also be reported to the charge nurse.

Celebrating 100 Years of Caring – Way Back Wednesday
In April 1979, the original Valley Memorial Home on 1023 Almonte Avenue was the temporary home for residents from St. Anne’s Guest Home while their building was evacuated due to local flooding. Cots were set up in a large room to accommodate 40 residents. Residents of St. Anne’s joined with residents of Valley in daily activities including church services, bingo, and birthday parties. St. Anne’s residents were enjoying themselves, including Lloyd who states, “I was tickled to death to get here… we like it here, while we’re here.” Later Sister Rebecca Metzger, Administrator of St Anne’s, wrote an editorial stating “The staff at Valley Memorial Home deserves a special salute for their hospitality and active concern. It is no easy task to take on a group of about 40 people without disrupting daily routines.” Residents of Tufte Manor continued providing for others in the community during the flood season by taking a collection of money and purchasing 1,025 hamburgers for those who needed food and had enough money left over to buy hot cocoa for flood fighters. It is uplifting to find examples throughout the years of Valley providing care to the community in a variety of ways!

Congratulations Dr. Henderson!
Our Medical Director Dr. Chris Henderson has achieved certification as a Certified Medical Director (CMD) in Post-Acute and Long-Term Care as of June 30, 2024! Dr. Henderson joins four physicians from North Dakota and more than 5,000 physicians nationwide who have received the CMD designation since the program’s inception in 1991. With the role of Medical Director being central to the delivery of quality care, this certification reinforces the leadership role of the medical director and serves as an indicator of professional competence to post-acute and long-term care providers, government, and other quality assurance agencies, consumers, and the public. Congrats Dr. Henderson, CMD!
2024 Silver Quality Awards
Congratulations to Valley on Columbia, Woodside Village, and Tufte Manor who have been recognized as 2024 Silver Achievement in Quality Award recipients by the American Health Care Association and National Center for Assisted Living (AHCA/NCAL). This award is the second of three distinctions possible through the AHCA/NCAL National Quality Award Program which recognizes organizations that meet progressively rigorous standards of performance to further improve the lives of residents and staff in long term care. Earning the Silver Achievement in Quality Award is an incredible honor for the teams at each care community and it is a testament to the quality of care and service we provide to the residents each day! Valley on Columbia, Woodside Village, and Tufte Manor are three of five care communities in North Dakota to be recognized this year with this award.

QAPI Update – Survey Revisit at Valley on Columbia
On July 2, a surveyor from the North Dakota Department of Health was onsite at Valley on Columbia to conduct a survey revisit. Since we received a G level deficiency, the revisit was conducted onsite instead of a paper compliance revisit that would typically occur. The purpose of the revisit was to ensure we are in substantial compliance with our plan of correction. The surveyor completed her tasks and no deficient practices were observed. Valley on Columbia is now back in compliance with the areas cited during the annual survey back in May. Auditing will continue to ensure continued compliance.

For July’s Wellness Challenge, the Wellness Committee invites you to participate in a Hydration Challenge!
Hydration plays a crucial role not only in physical health, but also mental well-being. Studies have shown that mild dehydration can alter mood and increase feelings of anxiety and irritability. Maintaining proper hydration levels is essential for mental clarity and emotional stability. Because of this, ensuring you drink enough water in your daily routines is a fundamental aspect of mental health self-care.
Instructions about the challenge can be found on the Hydration Tracker. Individuals who meet the hydration goal for at least 15 days out of the month will be eligible to enter a drawing for the following prizes: 1–$150 VISA gift card, 5–$50 VISA gift cards.

QAPI Update – F-Tag of the Week: F689 Free of Accident Hazards/Supervision/Devices Part 2
Over the next few weeks, we will be reviewing the F tags that were cited during Valley on Columbia’s survey. This week we are reviewing F689 which states that the resident environment remains free of accident hazards as is possible and that each resident receives adequate supervision and assistance devices to prevent accidents. The second part of this deficiency was related to a resident who was care planned to be supervised during meals. A supplement shake was provided to the resident for a morning snack and left unattended.
What you need to know: the snack sheets are being updated to include the level of assistance, as needed, for each resident. If a resident is completing the task at a level that differs from the plan of care, the charge nurse or care coordinator should be notified.

Happy Juneteenth!
Our newest national holiday is celebrated today throughout the country, recognizing the end of slavery in the United States. Today is the day in 1865 when the Union army entered Galveston Bay, Texas and the last people within the country were freed by executive order. While this or other versions of Emancipation Day have been celebrated throughout many areas of the United States for over 100 years, it is pretty new to us in our area of the country. Our country’s pursuit of ending slavery ultimately resulted in 620,000 deaths, killing about 2 percent of the entire population during the Civil War. More people died to create freedom for all in the United States than in all wars from the American Revolution through the World Wars and the Korean War combined.
Celebrating 100 Years of Caring – Way Back Wednesday
Valley Memorial Homes broke ground in October of 1979 on the current Valley Senior Living on Columbia location. It was originally known as the Valley Memorial Homes Medical Park skilled nursing facility. By way of the leadership and administration of Paul Opgrande, this expansion of services increased our capacity to care for 450 residents.

The first part of this deficiency was related to a facility reported incident (FRI) where a CNA failed to use a gait belt as care planned to assist a resident which resulted in a fall with fracture.
What you need to know: Gait belts must be used according to each resident’s plan of care. Each resident’s care needs can change frequently; the Kardex needs to be viewed prior to assisting a resident to ensure you are assisting each resident according to their current plan of care. If a resident is completing the task at a level that differs from the plan of care, the charge nurse or care coordinator must be notified.

Upcoming UKG Training
Over the next several months, Valley will be integrating to UKG, a leading provider of Human Resources and Workforce Management solutions, replacing the following applications with UKG’s equivalent: Payroll Professionals (HRIS), Orbital (Scheduling), AOD (Time & Attendance) and PlanSource (Employee Benefits Platform). Connie Miller, a UKG Consultant from HRMS, will be offering several live training sessions for employees and managers as well as a recorded session to view on HealthCare Academy for anyone who cannot attend a live session. Please plan to attend one of these sessions so that you can gain the necessary knowledge and understanding of these solutions, how they will impact you, and how you can navigate and utilize the system to the best of your ability!

Update to Employee Gift Policy
In support of the relationships built between staff, residents, and family members, Valley understands that residents or family members may want to give gifts to employees to express their appreciation. The gift policy was recently updated with the following changes:
- Employees may accept gifts of $10 value or less without the need to report the gift, an increase from $5.
- Employees may accept gifts of $10-$30 value but need to report the gift to their supervisor and complete a form before the end of their shift, an increase from $25.
- Gifts of value over $30 may not be accepted by an individual but instead must be shared with a neighborhood, department, or team.
If you receive a gift from a resident or family member and are not sure if you can accept it or not, check with your department supervisor. If you have any questions regarding the updated gift policy, please talk to your supervisor or HR.

Log your servings of healthy foods, track your minutes of physical activity outside of work, document your meaningful conversations, and record your hours of restful sleep. QAPI Update – F-Tag of the Week: F686 Treatment/Services to Prevent/Heal Pressure Injuries
Over the next few weeks we will be reviewing the F tags that were cited during Valley on Columbia’s survey. This week we are reviewing F686.
F686 states we must ensure that residents receive care to prevent pressure injuries and for residents with pressure injuries we must provide the necessary treatment and services to promote healing and prevent infection. The intent of this requirement is to ensure, to the extent possible, that residents do not develop pressure injuries unless they are clinically unavoidable and that staff provide appropriate care and services to prevent injuries, promote healing, and/or prevent the development of additional issues.
What you need to know: In order to prevent pressure injuries and promote healing, residents must be repositioned and pressure relieving/reducing interventions must be in place according to their care plan. Interventions may include wheelchair cushions, air mattress, protective boots, etc. If a resident is choosing to decline any care planned tasks/items, you must chart this and report to the charge nurse. Any new reddened or open areas should be reported to the charge nurse.

UKG Training Options Begin Today (6.11.24) – Please plan to attend!
As we prepare for the launch of the UKG software within the Human Resources department we recommend employees attend a training session to equip employees with the knowledge and skills necessary to navigate and optimize our new systems. By partnering with UKG, we will be replacing the following applications with UKG’s equivalent: Payroll Professionals (HRIS), Orbital (Scheduling), AOD (Time & Attendance) and PlanSource (Employee Benefits Platform). With UKG’s solutions and technology, you will be able to access a wide range of HR information conveniently, right at your fingertips! There are three Live virtual sessions for employees to attend, and three Live virtual sessions for managers to attend. For those unable to attend one of the Live virtual sessions, a pre-recorded version of the training will be placed on HealthCare Academy for you to review and complete.

Celebrating 100 Years of Caring – Way Back Wednesday
Our corporate congregations have supported Valley in numerous monetary ways, through physical items, volunteerism, and leadership by determining the direction of our organization over the past 100 years. In 1960 Bygland Lutheran Church in Fisher gifted an altar to the original Valley location at 1023 Almonte Avenue. This altar was an area where many residents worshiped for years. After the flood in 1997 that altar was moved from the Almonte location and later placed at Valley Eldercare Center in the meditation room. The support of our corporate congregations has been an integral part of our organization for 100 years!

Valley on Columbia Survey Update
Valley Senior Living on Columbia had our annual licensure surveys for health and life safety during the week of May 6. Our life safety survey which looks at different elements of our building systems and emergency plans resulted in no deficiencies. Yesterday we received our report on the health survey which resulted in six deficiencies: develop/implement comprehensive care plan, treatment/services to prevent/heal pressure ulcer, environment is free of accident hazards/supervision/devices, bowel/bladder incontinence/catheter/UTI, hospice services, and infection prevention and control.
All six deficiencies were isolated in nature and involved one or two residents or one or two staff. The average number of deficiencies in the country is 9.4 and in North Dakota it is 5.3 deficiencies per survey. With 35 residents in the survey sample, six deficiencies is not a poor survey, but we are disappointed because there are more deficiencies than we typically receive.
The process at this point is to send individual plans of correction for each deficiency to the ND Department of Health. They will accept or recommend a modification and then our survey is finalized. It is followed by a “revisit” which is a process to make sure that our plans of correction are in place and improvements are sustainable.

Bring Your Child to Work – Volunteering!
With school out for summer, it is exciting to consider how we can get our kids more involved with residents throughout our care communities. Youth are invited and encouraged to participate in various resident activities through Bring Your Child to Work. A few guidelines are in place to help this be successful for all involved which are outlined here. Applications for youth volunteers need to be filled out and signed by your supervisor. You can return completed applications to Kayla Gust, Event and Volunteer Coordinator. Each youth volunteer will receive a name badge and a pin number to login/logout on the volunteer tablet each time they are here. QAPI Update – F-Tag of the Week: F805 Food in Form to Meet Individual Needs
F805 states that each resident receives food prepared in an appropriate form to meet their needs based on their care plan.
What you need to know: The Kardex must be reviewed prior to serving food to residents. Serving the incorrect diet could have significant negative consequences to the resident including choking, developing an infection, or a transfer to the hospital. The correct adaptive equipment must also be used. You need to report changes in a resident’s ability to eat and drink according to their care plan to the charge nurse. Examples of changes may include coughing, choking, holding food or drinks in their mouth, or decreased consumption.
Refer New Employees to CNA Training Class and Earn a Recruiter Bonus
The next CNA Training Class is scheduled to begin on July 8. A recruiter bonus remains available to employees that recruit and refer new care partners to Valley Senior Living. You can earn $1000 for each full-time employee you recruit or $500 for a part-time recruit. Once you refer someone and they are hired, contact Human Resources to complete the recruiter bonus paperwork.

Applications for the Greg V. Hanson Scholarship are due to Human Resources by June 22. This scholarship is for Valley Senior Living staff who have been employed at Valley for at least three years and who have not been accepted to nursing school at this time.
