Open Enrollment for Benefit Election is now open!

Open enrollment for Valley Senior Living benefit election is open, November 17, 2023 through December 1, 2023.

Please be sure to access the PlanSource platform link below and acknowledge what benefits you would like to elect to receive for 2024.

For those of you unsure of your username and password, please see below for an example of what it may be:

Username: First Initial, Up to 6 digits of your last name, Last 4 digits of your Social Security Number (SSN)

  • Example: Don White – SSN 1234, Username: Dwhite1234

Password: First Name, Hyphen, Last 4 digits of your Social Security Number, 3

  • Example: Don White – SSN 1234, Password: Don-12343

  • Contact Information for BCBS Representative to assist staff with questions surrounding prescriptions.
    • Important Note: When calling Gina or Michael, please have the policy number of the plan you are thinking of enrolling in. This policy number can be found in the Employee Benefits Guide. This number will assist Gina and Michael by providing you with specific information for that policy as it relates to your prescriptive medication questions.
