Transitional care provides post-acute skilled nursing care and rehabilitative services for individuals to successfully return home or to an alternative level of care. Our focus with transitional care is to maximize recovery by tailoring a plan of care that will meet the needs and goals of each individual.
Residents can receive ongoing IV therapy, wound care, physical therapy, occupational therapy, speech therapy, and respiratory therapy. Our therapy services are provided by skilled therapists that focus on functional resident goals and use a variety of equipment and modalities as part of their daily treatments.
