Valley Transitions is a rehabilitation community within Valley Senior Living on Columbia. When we say we provide transitional care, we mean our goal is to get individuals back home or to an alternative level of care. At Valley Transitions, our focus is on maximizing recovery, so we tailor our care plans to meet the unique needs and goals of each individual.
Valley Transitions has 30 private and three semi‑private rooms, each recently renovated to create a comfortable, home‑like space. Every private room includes a bathroom with an accessible shower, a flat‑screen television, a mini fridge, and Wi‑Fi, giving residents convenience and comfort.
Our care partners work closely with our Medical Director and provider team to create personalized care plans for each resident’s recovery.
Transitional care is for individuals recovering from illness, surgery, or life-altering events who require specialized care before they’re able to return home or transition to a higher level of care.
